If you have goods to return to Jacques, please complete the ‘Return Material Authorisation (RMA) Form’ below. Completing all the required fields will give us the necessary information to quickly and efficiently process your service request. 

Please note: American Express and Diners Club are not accepted, payment must be via Mastercard or Visa.

RMA PROCESS

Once you have completed and submitted the RMA form, you will receive an email giving you further instructions for their return. Ensure you include a copy of your RMA email with the returned goods. Goods returned without the required paperwork cannot be processed.

Ensure you include PO number for the required work. Forms submitted without a PO number will delay Jacques repairs. 

REQUEST FOR QUOTE

If you require a quote prior to the commencement of repair work, there will be a $75 upfront fee. This fee is payable by credit card or by approving this fee be added to a valid PO before the RMA can be submitted.

TIME FRAME FOR REPAIRS

There is typically a two week turnaround from the time we receive your goods (with correctly completed RMA form) to the completion of the repair work.
